Job Description

The Nurse Coordinator is responsible for the coordination of the activities for the division/program. The Coordinator will provide outpatient nursing care and patient/family services on an initial and ongoing basis. The Coordinator will serve as the patient/family’s initial contact with the program and handle all new referrals with appropriate scheduling, program planning and enrollment.

Job Responsibilities

  • Receive all new referrals for the (insert name) division/program. Communicate and coordinate with referring medical personnel / agencies / parents or caregivers. Send follow up letters for patients who are to be enrolled.
  • Schedule and organize medical clinics / evaluations with Director and other providers.
  • Review and edit billings to outside agencies to ensure accuracy and verify the provision of services.
  • Review evaluations / re-evaluations and update reminder list with the recommended medical follow up.
  • Coordinate services with allied health as necessary.
  • Act as a liaison with other community agencies and provide input from the community in program planning.
  • Request and schedule necessary therapy, educational evaluation and treatment times.
  • Oversight of the planning, implementation and evaluation of the DSME Program.
  • Arranges and coordinates the activities of the advisory group.
  • Liaises between the staff, the advisory group, other departments and administration
  • Monitors and facilitates maintenance of staff qualifications.
  • Assess, plan, implement, and evaluate the educational needs of diabetic patients.
  • Implement an individual teaching plan, coordinate continuing education, and provide follow-up diabetes and health care management.
  • Provide initial and follow-up insulin pump education, CGMS, and glucometers.
  • Provide management via internet and telemedicine for diabetes and gestational diabetes patients.
  • Conduct diabetes self-management education (DSME) group classes.
  • Assist with developing a comprehensive AADE /ADA accredited program in diabetes education (DSME) under direction of endocrine faculty.
  • Conduct specialized endocrine tests with repeated blood sampling.
  • Schedule and organize Nuclear Medicine/ Endocrine tests including radioiodine scans.
  • Obtain prior authorizations and provide prescription refill support.
  • Communicate verbally, and via electronic medical record, to health care team, goals of treatment as needed.
  • Document all plans of care and record patient meter readings in medical chart.
  • Answer telephone inquiries.
  • Adhere to timely documentation using electronic medical records.

Additional Job Description

MINIMUM EDUCATION & EXPERIENCE

  • BSN (required)
  • Current Kentucky Nursing License (required)
  • Four years’ experience as RN; prefer experience in a medical office setting
  • Supervisory experience
